Tandigm Health Signs Landmark Agreement with Holy Redeemer Health System and Innovative Wellness Alliance

, Holy Redeemer Health System, and Innovative Wellness Alliance (IWA) today announced a landmark agreement to improve the delivery of quality health care, reduce costs, and enhance patient satisfaction. Under the agreement, more than 75 primary care physicians from IWA, a physician-led, patient-centered organization, along with Holy Redeemer’s extensive network of inpatient, outpatient, home health, and long-term-care services, will collaborate with Tandigm Health and its growing network of primary care physicians. These physicians will benefit from Tandigm Health’s clinical expertise, analytic tools, and real-time data to help them better manage difficult chronic conditions, expedite preventive care, and deliver more cost-effective, high-quality care. As a result of this collaboration, Tandigm will have a network of more than 350 primary care physicians.

The deal marks Tandigm Health’s first cooperative arrangement with both a clinically integrated network of primary care physicians and a health system that provides a full continuum of healthcare services. The agreement will be effective March 1, 2015.

Tandigm Health, based in West Conshohocken, Pa. was formed in April 2014 by affiliates Independence Blue Cross and DaVita HealthCare Partners (NYSE: DVA). IWA was formed in January 2014 as a clinically integrated network comprised of primary care physicians, nurse practitioners, and Holy Redeemer to improve quality and manage the cost effectiveness of care. Holy Redeemer is uniquely qualified to deliver excellent care in the most appropriate and cost-effective environment because it has facilities and services that cover the continuum of care from in-patient hospitalization to home care to long-term care.
